LabCard is still also available for those participants that live in a location where LabCard works well, please continue using this valuable program if you wish. LabCard providers are located in several Oklahoma and Texas cities.

• Go to www.labcard.com. You may search by state for a list of all LabCard providers in your area or you may enter your zip code to see the closest provider.
• Present your Advantage Health Plans ID card and your physician’s lab services prescription.
